GPMS TB Transportal
G P M S
The digital solution to track the missing million
Indian Centre for Social Transformation(Indian CST) believes in ‘IT for social change’ and strongly supports the National vision of a TB free India.
The Made in India Global Project Management Software for TB(GPMS TB Transportal (https://indiancst.com/India/universalhealthcare/index.php) developed by Indian CST provides integrated system level platform connecting TB patients with healthcare service providers for identifying high density areas, individual case data(linking to Nikshay or other relevant Government portals).Digitization paves the way for integration of State and National data for policy decision making and accountability.
Implementation modalities
The transportal combines social, technological and analytical innovations and acts as an exclusive GPMS TB-portal that connects patients through audio/video channels. This multilingual platform uses indigenous video technology which has the capability of capturing cough episodes of TB patients using a mobile phone. Door step Mobile X-rays facility is also available which are then analyzed remotely. All this has resulted in reducing patients’ hardships and avoided loss of wages to them by creating a model for Remote Virtual Patient Connect and Voice Enabled Technology. Patients’ data captured on the transportal cloud helps in checking dropouts and further tracking / follow-up to ensure better treatment compliance. The bilingual APP version is compatible on android tablets and smart phones which can utilised by ASHA workers at the grassroots.
The transportal uses direct cloud solution in order to keep the data in Indian Government cloud at CSIR-4PI.
Features of the Transportal
- Strengthening ACF:
- GPMS TB portal collects a critical range of real time data for active case findings(ACF) and presumptive TB which would help in identifying the vulnerable population in higher proportions than identification through passive case finding and is proposed in the TB control program in the country
- The GPMS TB transportal has enabled digitalisation and cloud based data reporting capabilities at the grass route level. It has made possible undertaking surveillance functions such as reporting, data analysis , feedback in real time manner at the grassroots
- Improvement of Direct Benefit Transfer (DBT):
- The major issue faced by the public health sector is the inefficient utilisation of allocated funds. It requires multiple approvals and paper based documentation, over burdened staff, technological challenges leading to substantial delays. GPMS TB portal enables the collection, storage , use and dissemination of patients digital health information which includes Aadhaar number, demographic and biometric information.A coordinated system between GPMS, Nikshay and PFMS will enable prompt and speedy enrolment and social support services for the beneficiaries
- GPMS real time tracking is more outcome based supplementing national data like Nikshay, NSTOP and Nikshay Poshan Yojana. The impact can be captured by the surveillance system and can be amplified and ineffective ones tapered off.
- Coordinated TB surveillance:
- It significantly reduces inaccurate reporting and ill-informed treatment administration, especially in areas with variable TB transmission patterns
- Enhances the predictability of TB occurrence and its transmission; thereby alerting public health officials to indicators of elevated TB risk
- It has the potential to be an integral tool for health officials, analysts and decision makers
- Tracking and tracing is very difficult in paper based format
- Issues with Xray readings and multiple data entries in physical system
Backlog of previous reports and dashboard indicators in the existing system
Our milestone projects
Digitization & ACF across 4 districts in Karnataka
Indian Centre for Social Transformation was given the mandate to conduct pilot studies and provide capacity building for the digitization of Active Case Finding (ACF-Tuberculosis) for High Risk Taluks (Bengaluru Urban, BBMP, Tumakuru, Chikkaballapura, Kolar) in 4 Districts of Karnataka using GPMS Transportal for Universal Health Care with the support and guidance of Dr.Manjula, Joint Director State TB Cell, Karnataka Commissioner, Department of Health and Family Welfare Services, Government of Karnataka, Mission Director NHM, Karnataka and Deputy Director (e-Hospital)
SOCIAL IMPACT
This initiative helped in digitizing 6,00,000 TB case records identified by the NTEP in the past thereby leading by example that digitising can help reduce carbon footprints and digitalisation can help in transforming this data into effective programs on a large scale
- Integration of 10 years of data
- Capacity building and Training programs for 32000 ASHA Mentors, ASHA workers, STS, STLS, Lab Technicians and medical students in the State of Karnataka for the utilization of the Mobile/Tab based App customized and developed by Indian CST
- Identification of 5343 TB presumptive on a large scale and notifying them into Nikshay
World TB Summit
Presented the ACF digital survey study, Karnataka project at the World summit TB with further recommendations to implement this unique digital platform to be replicated in other States.
SOCIAL IMPACT
An integrated system level software connecting TB patients with healthcare service providers in the state of Karnataka with real time data from 9130 subcenters / 2523 PHC’s/ 146 Taluk hospitals/173 TB units further demonstrating and validating Indian CST’s vision( Article 51A)of our Indian Constitution of collective and collaborative efforts to achieve higher levels achievement and endeavour.
Our successful initiative for a TB mukt Bharat in the State of Karnataka led to a joint venture with ICMR and private hospitals for furthering our reach to find the missing millions in high risk population of migrant workers and rickshaw pullers.
TB prevalence and intervention for reducing TB and LTBI in the high risk population of rickshaw drivers(Delhi) and construction workers(Bangalore)
GPMS TB transportal – Universal HealthCare platform was replicated for an ICMR project to find the missing millions. Indian CST with an expertise in digital technologies partnered with M.S.Ramaiah hospital, Karnataka State Health TB officer-Bengaluru and Share India, Delhi ; initiated the TB survey and TST administration at 35 construction sites, organising training programs and workshops for capacity building, etc which establishes and validates our efforts even more.
SOCIAL IMPACT
8000 construction workers and rickshaw pullers from Bangalore urban and Delhi were screened for TB and TST was administered. 200+ TB presumptive cases were identified and notified into Nikshay for further diagnostics and treatment.
Scientific Research and Publications:
Indian CST has been involved in scientific research & publications for evidence based practice to fight this disease and help in addressing the poor man’s disease through Open Source Drug discovery and evidence based global digital solutions..
- Research: A study on “Structure based drug discovery for designing leads for the non-toxic metabolic targets in multi drug resistant Mycobacterium tuberculosis”, S.K.Bramhachari et al(J Transl Med 15, 261 (2017). https://doi.org/10.1186/s12967-017-1363-9)
Outcome:
- Identification of 20 novel lead molecules including 4 US FDA approved drugs which can be further taken for drug repurposing
- Tackles the MDR form of TB and persister population of the bacterium, with a potential to reduce the failures in the Tb drug discovery
- Publication: Geno-Cluster A novel platform software tool for facilitating new drug discovery
A joint initiative developed by Institute of Genomics and Integrative Biology (IGIB) and the Council of Scientific and Industrial Research (CSIR) and now further development and hosting it is supported by Indian Centre for Social Transformation (Indian CST) a public charitable Trust.
Outcome:
- US patented and have been installed in the leading academic and research institutions all over India and across the world
- The software has already proved to be of tremendous use in the discovery of novel genes of the SARS virus and has several papers credited to its findings
- Publication: “Envisioning a learning surveillance system for Tuberculosis(2020)”
An effective surveillance system is critical for interrupting the transmission of global epidemics and remains central to public health systems. Results have shown that GPMS TB transportal collects a critical range of data for ACF’s and presumptive case screening for identifying and detecting confirmed TB cases.
Outcome:
- Recently published on PubMed, National Library of Medicine-NCBI, 2022( Suresh V. Et al(PLoS One. 2020 Dec 14;15(12):e0243610. doi: 10.1371/journal.pone.0243610. PMID: 33315902; PMCID: PMC7735594)
